If not in your bags, then abandon quest, remove costume buff, click on table for the ingredients, talk to Thelaron Direneedle to get costume, and then use the item. Remember the quest A Perfect Costume? You can not get the disguise anymore once you turn in the quest, Locked and Loaded. Comment by tirsden A friend lost her disguise and it actually became an invisible icon, she found it by searching through her bags and saw the mouseover text when she passed an empty slot. She could use it as normal by right-clicking the empty space, just couldn't see the actual item in inventory.

Select "investigate the hides" then choose "take three hides" followed by "take two handfuls of mud" followed by "Take one spool of thread" and then talk to Thelaron. Investigate the hides, then choose two hides, then choose four handfuls of mud, and then choose two spools of thread. Afterwards, click on Thelaron Direneedle and he should give you the disguise. So i back to my messy bank I click on search button and search the word Blackrock disguise and i see its there on my back and that's why i cant speak to npc again for new one.
